Understanding the 1st January 2024 Public Health Sector Pay Scales Circular in Ireland

The Department of Health in Ireland has issued Circular 2/2024, dated 3 April 2024, which outlines important updates and adjustments to the salary structures across various public health roles, effective from 1 January 2024. This circular aims to inform healthcare providers, HR managers, and public health employees about recent pay scale revisions, including specific recommendations for nursing, midwifery, medical scientists, and senior management roles within the health service executive (HSE).

Scope and Application of the Circular

This circular applies primarily to employees within the public health sector managed by the HSE, including nurses, midwives, medical scientists, and chief officers. It provides detailed instructions on how existing pay scales are to be adjusted and implemented retrospectively from 30 September 2023, with specific focus on certain grades and allowances. The document also covers the alignment of pay scales for senior medical roles and chief officers, ensuring transparency and consistency in remuneration across the sector.

Key Pay Adjustments for Nursing and Midwifery Grades

Implementation of Recommendations from the Expert Review Body on Nursing

The circular details the application of recommendations 44, 45, and 46, which impact public health nurses and clinical nurse managers. These recommendations include:

  • Migration to New Pay Scales: Public Health Nurses (PHNs) and those newly appointed since 30 September 2023 will move to the Clinical Nurse Manager 2 (CNM2) / Clinical Midwife Manager 2 (CMM2) pay scale on a point-to-point basis.
  • Scale Extension and Rate Adjustment: The CNM2/CMM2 pay scale will be extended by an additional scale point, with the maximum point (Pt10) set at a rate of €69,136 as of 30 September 2023. An additional Local Scale Increment (LSI) at Pt11 will also be introduced, with a rate of €71,210.
  • Allowances Expansion: Eligibility for the Specialist Qualification/Location Allowance has been broadened to include specific grades such as Clinical Nurse Manager 3 (General, Mental Health, Night, Theatre) and Clinical Midwife Manager 3, effective from 30 September 2023.

Retrospective Application and Employee Transition

Employees currently at the ninth point of the pay scale will be transitioned according to their length of service:

  1. Employees on Pt9 for one or more years will move to Pt10.
  2. Those on Pt9 for four or more years will progress to the new LSI at Pt11.
  3. Employees with two or three years on Pt9 will move to Pt10 and reach the LSI after two years, respectively.

The circular emphasizes that these adjustments will not negatively impact incremental progression or existing service credits, and overtime or allowances will be calculated based on the revised pay rates.

Pay Scale Alignments for Senior Medical and Management Roles

Medical Scientists and Related Grades

The circular specifies that Medical Scientists (grade code 3875) will be aligned with Biochemists (grade code 3751), and Senior Medical Scientists (grade code 3877) will be aligned with Senior Biochemists (grade code 376X). These changes include removing qualification barriers and allowing progression along the new scales from 1 January 2024.

Chief Medical Scientists and Chief Officers

Chief Medical Scientists (grade code 3876) will be paid on a scale comparable to Principal Biochemists (grade code 3778), with progression up to a specified point contingent upon possessing relevant qualifications such as a fellowship or postgraduate degree. Similarly, Chief Officers serving as Community Health Organizers (CHOs) will be aligned to the pay scale of the HSE National Director 3, effective from 1 December 2022, with retrospective effect.

Implications for Public Sector Employees

This circular ensures that pay adjustments are implemented fairly and transparently across the public health sector, reflecting recent recommendations and sectoral restructuring. It provides clarity on the retrospective nature of these changes, the criteria for progression, and how allowances and overtime are to be calculated, thereby supporting employees and HR departments in managing payroll adjustments effectively.

Overall, Circular 2/2024 signifies a significant step towards harmonizing pay scales, recognizing service and qualifications, and aligning remuneration with sectoral standards in Ireland’s public health system for the year 2024.
